Search Criteria (Provider)
When searching the database to determine if a Provider has been previously entered, the object is to use a greater number of search parameters to yield the smallest number of Providers matching the search criteria.
Provider ID Number
Enter the system-generated Identification Number of the provider in this field.
Program Name or Last Name
Enter the name of the program (Licensed Provider) or the last name of the Provider (Registered or LECC).
First Name
Enter the first name of the Provider (Registered or LECC).
License Number
Enter the license number assigned to the Provider.
Provider EIN/SSN
Enter the Federal Tax Identification or Social Security Number associated with a provider in this field.
Town
Select the name of the town from the drop-down list using the down arrow next to this field () to restrict the search parameters.
Provider Type
Select one of the following from the drop-down list using the down arrow next to this field ():
Family Child Care Program
Early Childhood Program
Non-recurring Client
School Age Program
Registered Family Child Care Home
Legally Exempt Child Care
License Status
Select one of the following from the drop-down list using the down arrow next to the each of the First and Last name fields () to restrict the search parameters:
Applicant
Denied
Expired
Intent to Deny
Intent to Revoke
Intent to Suspend
Licensed
Pending
Revoked
Suspended
Withdrawn
Application Confirmation ID
Enter the number that has been system-generated for the Application Confirmation Number.
