Section 1: Applicant Information (Search and Validate)
As part of entering information about a new applicant, it is imperative that the new applicant's name be unduplicated in the system. In order to accomplish this, the Search and Validate page (LI0007) is incorporated into the flow of entering Initial Licensing Visit Request provider information. People and organizations entered as part of the Initial Licensing Visit Request of the Application process are screened against Bright Futures people and organizations, and demographics are updated as required.
Enter as much information as you want in the fields presented in order to limit your search results. The more information entered, the less results will be returned.
Name (Last*/First)
Enter the name of the applicant in these fields. If the provider is an organization, enter the name in the Last Name field.
Search Type
Select one of the following from the drop-down list using the down arrow next to the each of the First and Last name fields () to restrict the search parameters:
Sounds Like
Starts With
Exactly
Contains
Gender
Select Female or Male by clicking in the () to the left of the option.
Date of Birth
Enter the date of birth using the following format MM/DD/YYYY.
Reference ID
Type the identification number to be used with the Reference Type in this field.
Reference Type
If a number has been entered in the previous field, select one of the following for the associated type:
CCIS CCIS License ID
DOE Department of Education Student ID
DMV Driver's License Number
EIN Federal Identification Number
SSN Social Security Number
ISN PATH ID
SSID Social Services ID
SEID State Employee ID
VID Vendor ID
Once the information has been entered in the above fields, click on the Search button to perform a search in the database for an existing provider.
