Complete the following information to finish this section:
Application Date
Enter the date of the application using the date format MM/DD/YYYY.
Parent Statement of Understanding
Enter the Signature Date for the statement. This date indicates that the parents are aware of the terms of the LECC application.
Click on Next to advance to the next section of the application.
