Section 2: Need for Care/Special Need (SB0039)
The Need for Care Questions identifies the subsidy applicant's activities which constitute the reason(s) for needing subsidized child care. Enter the information about the need for care and special needs in the following sections.
Primary Service Need*
More than one Service Need may be entered, but one and only one may be designated as Primary. If Special Health Need/Disability Parent is indicated as the Service Need, no schedule of activity is required for that parent. A child schedule would still be required.
As a result of an application created from either a PATH Authorization or Protective Services Referral, the Primary Service Need has been pre-filled with the following values:

Employment Employment
Postsecondary Education Education
Reach Up Activity Training
These values should not be changed because the values are used during payment processing to determine the funding for the subsidy.
For all other applications, select the value by using the down arrow () next to the field.

Service Need
In addition to the Primary Service Need, you may select as many other Service Needs as appropriate. Check each of the needs that apply by clicking in the box next to the value:

To uncheck a selected need, just click in the box again to remove the selection.
Stressful Situation Question*
Select either Yes or No for the selection by using the radio button for this field ().
Family Support Score
Upon review of the Family Support Questionnaire, a score is entered on the application. This score is used as criteria to determine eligibility for Family Support.
Child Only Case*
Foster children will constitute a "child only" case with identification of the appropriate foster parent. A foster child therefore comprises a family size of 1. Select either Yes or No for the selection by using the radio button for this field ().

Special Needs Begin Date
Enter the beginning date for which the special needs have been documented.
Special Needs End Date
Enter the ending date for which the special needs have been documented.
