Assign Provider Tier Level
In order to add a new tier level, click on the Assign Provider Tier Level link in the Quality Options area. As a result, the Assign Provider Tier Level page (QU0104) is displayed.
On this page, you may enter the information in the following fields:
Current Tier Level
The Tier Level currently assigned to the provider is displayed in this field and cannot be changed. In order to change it, enter the New Tier Level as described below.
New Tier Level*
Select one of the following values from the drop-down list using the down arrow next to the field (
):
Not Rated
1 Star
2 Star
3 Star
4 Star
Start Date*
Enter the starting date for the new tier level in this field.
End Date
Enter the ending date for the new tier level in this field. This field is not always completed.
