This page identifies the need for child care and parent/caretaker eligibility. Enter the information about the need for care and eligibility in the following sections.

Begin and End Dates*

Enter the beginning and ending dates for the authorization in these date fields using the format MM/DD/YYYY.

Authorization ID*

Enter the identification number for the authorization in this field.

Type*

Select the type from the drop-down by using the down arrow () next to the field.

Reason for Care*

More than one reason may be the circumstances for care, but one and only one may be designated as Reason for Care. Select the value by using the down arrow () next to the field.

Other Service Need

Indicate the service need by checking each of the needs that apply by clicking in the box next to the value:

Child Care Resource and Referral Services

To uncheck a selected need, just click in the box again to remove the selection.
