Site Visit Corrective Action
If violations are noted and corrective actions are required, these actions are tracked to their respective conclusions. Site Visit Corrective Actions are posted to the Corrective Action Tracker for review and follow-up by the Licensing Technician on or before the day that the Site Visit Corrective Actions are due for completion. Each corrective action has a due date and a completed date. The due date is established during the Site Visit and the Licensing Technician records the completed date. When the completion date is entered into the system, the Site Visit Corrective Action is removed from the Corrective Action Tracker. When all corrective actions have been addressed, the site visit is complete. For complete instructions on the Corrective Action function, see the Provider Management Chapter.
If violations are noted, the system generates a Provider Problem Notice identifying the problems. This notice is sent to the Licensing Supervisor and to the Licensor when they are not the worker documenting the findings. The notice may also be sent to the Subsidy case worker(s) who have children enrolled with the Provider if the violation(s) found involves parental notification.
For site visits related to a complaint, a finding of violations is equivalent to substantiating a specific complaint allegation and a finding of no violations is equivalent to un-substantiated allegation. A Complaint is not completed until the site visit associated with the investigation of the complaint is closed.
If no violations are found, this outcome is noted and the Site Visit is complete. A Site Visit Report is generated even if no violations are found.
The primary documentation of completion of corrective actions is the Corrective Action Certification, which is contained in the Site Visit Report and returned by the Provider to the Licensing Technician. When the Certification is received, it is used by the Licensing Technician to enter the Corrective Action completion date. When the certification is received, the Licensing Technician may enter the dates with no verification, or verify the actions by telephone. In addition, the Licensor may visit the site again to verify the corrective actions.
During the tracking process, multiple attempts to contact the Provider to follow-up on the violations and their resolutions may be necessary. During the follow-up period, the worker documents the contacts with the Provider in the Case Contact Log.
If corrective actions are not resolved after repeated attempts at resolution, the Licensor may recommend that a License Action be taken to suspend, revoke or close a license. These recommendations are handled through the License Action process (See Provider Management Chapter).
