Site Visit Report
Formal documentation of the Site Visit findings is completed in the system following the visit as described above. A copy of the Site Visit Report is generated by the system from this documentation and sent to the Provider. The Provider is asked to sign an acknowledgement of all violations and corrective actions noted in the report. The Provider then returns one copy to the Licensing Technician. Once all corrective actions have been completed, the Provider returns a second certification documenting these facts.
