Submitting the Initial Licensing Visit Request

An application is initiated by the submission of an Initial Licensing Visit Request (Visit Request or VR), accompanied by a Record Check Authorization.  For a new application, the record check authorization must be received in hard copy with a live signature.  The Request for site visit, however, may be submitted from the Internet.  The Internet version is accompanied by section-by-section instructions for the VR.  In addition, the Internet site will provide a description of the Licensed program process, a copy of the complete application, and instructions for completing the application.  As part of the VR, the applicant will be asked if they would like to become an eProvider, and send licensing forms and communications from the web site.  The system ensures that all required information for the VR is provided before permitting it to be submitted to the CDD Licensing Unit.  Upon completion and submission of the Visit Request, the following items are generated by the system:

      Provider Portal:  The Confirmation Number (Application Number) is assigned by the system and the number is displayed for the provider for reference use in submission of the hard copy Record Check Authorization. 

      Internal Portal:  Upon submission of the Initial Visit Request, a Confirmation Number (Application Number) is assigned by the system and displayed at the end of the process with a link to access the provider's account in order to continue with the Application process.

      Record Check work item is worklisted to a Licensing Technician (assigned through the Town/District mapping) for review and processing;

      A Licensing Case is opened and the License is in an "Applicant" status;

      The Provider Account Summary is established with the entered information and the entered information may be found on the Account Summary page (PM0083);

      The Applicant is recorded as an associated party for the account;

      The Licensing case is assigned to the Licensor through the Town/District mapping; and,

      A Case Assigned Notice is generated for the Licensor to inform him/her of the Case assignment;

      Upon a positive Record Check outcome for the Initial Applicant, a Site Visit Required Notice is generated for the Licensor; and,

      A Licensed Application is available for completion on the Account Summary page (PM0083).
