Record the information on the Change Provider Status page (LI0174) as described in Starting the Provider License Status Change.
Once the information has been entered, click on the Continue button to submit the License Status Change.
In this case, since a previous entry had been recorded for a Provider Violation, the next page displayed is the Change Provider License Status - Regulatory Information page.
The Provider Violations table would present the list of violations and a selection may be made from the table by checking the row in the Select column and then by clicking on the Use Selected and Continue button. This violation will become a part of this particular License Action.
If you do not wish to attach a previous violation, clicking on this button will simply take you to the next page with no violation listed. As a result of this action, the next page displayed is the Change Provider License Status - Create Violations page (LI0143).
You may add violations, if appropriate, or you may Submit this page with only the selected violation(s).
