Transportation Information

Complete the information in the following fields:

Contract Type

Select Formal or Informal by clicking in the () to the left of the option.

Towns Served

Select as many towns for this field as appropriate.  In order to select multiples, hold down the CTRL key as you use your right mouse button to highlight and click on the selection.

Number of Children Served

Enter the number of children served for the transportation agreement in this field.

Mileage Indicator

Select Yes or No by clicking in the () to the left of the option.

Mileage Reimbursement Amount

If the answer to the Mileage Indicator is Yes, enter the amount for the reimbursement in this field, using the format $. ¢¢. 

DMV Check Performed

If a Division of Motor Vehicle check has been performed, click on the box next to the value to indicate this.
