Viewing Details for the Transportation Authorization
In order to view the details for a particular entry in the table, click on the active link for the Authorization ID for that row. As a result, the Transportation Authorization Detail page (SB0109) is presented.

Provider
Provider ID
Start Date
End Date
Days Authorized
