Change Provider



In order to change a provider for an enrollment, follow these steps.

Click on the Certificate ID on the Eligibility page.

This accesses the Certificate page.

Click on the End Certificate link at the bottom of the page.

Complete New End Date.  Note that system edits require that this date is today or a future date (due to attendance and payment processing).  Fill in the End Reason.  Click on the Submit button.

Once this certificate has been ended, proceed to enroll the child with the new provider.

On the Eligibility page, click on the New Certificate link for the Authorization of the child making the change.

The Create Certificate - Link to Provider page (SB0042) will be displayed.  

Perform a search for the new provider and continue to create the certificate as described in Continuing with the Enrollment. 

On this page, enter the Start Date (this date has to be the day following the previous Certificate End Date).  Complete the remaining information on this page.  Click on the Submit button.
