Complaint Search


The Complaint Search allows workers to search for validated complaints meeting their specified parameters.  Use as many fields as known in order to limit the search results.  The following search fields are available.

Complaint No.

If you know the Complaint ID Number, enter it here.  This entry would give you the most direct connection to the entered complaint.

Complaint Type*

Select Provider or Subsidy by clicking in the () to the left of the option.

Complaint Sub-Type

Select the value for this field by using the down arrow () next to the field.

Subsidy

      Denial

      Regulatory

      Termination

Provider

      Business Practice

      Chapter 49 Abuse or Neglect

      Non-Regulatory Relationship with Parent

      Payment Issue

      Regulatory

Subject Last Name or Provider Name

Enter the subject's last name in this field.  If the provider is an organization, enter the provider name in the Last Name field.

Subject First Name

Enter the first name of the person that is associated with the Last Name field described above.

Complainant Last Name or Provider Name*

Enter the complainant's last name in this field.  If the provider is an organization, enter the provider name in the Last Name field.

Complainant First Name

Enter the complainant's first name in this field.

Date Received (From and To Dates)

To view complaints that were created on a specific date or within a specific date range, use the Date(s) filter.  The date must be entered in MM/DD/YYYY format.  To view all complaints created after a specific date, enter that date in the Start Date field.  All complaints created after the entered date will be displayed.  To view complaints that were created between a certain date range, enter a date in both date fields provided.  Only complaints created between the specified date range will be displayed in the Results table.  To view complaints that were created before a specific date, enter that date in the End Date field.
