Complete Application for Licensed Program


For those who have elected to become an eProvider, following the site visit the applicant completes the Licensed Program Application from their Internet site.  As with the original Initial Licensing Visit Request, the Internet version is accompanied by section-by-section instructions for completion of the application.  The system ensures that all required information for the application is provided before permitting it to be submitted to the CDD Licensing Unit.  Upon completion and submission of the application, it is worklisted to a Licensor for review.
