Completing the Child's Record
After selecting one of the options, Use Selected or Create New Party, the next page presented is the Add Child page (RR0026) shown below.
Complete the information for the following fields.
Last*, First* and Middle Names and Suffix
Enter the name of the child in the appropriate name fields. If you have selected a previously entered party from the Search Results page, this name will be carried forward from that selection.
Date of Birth*
Enter the date of birth using the following format MM/DD/YYYY.
Gender
Select Female or Male by using the radio button next to the field (
