Completing the Contact Log Entry


Once you have accessed the Contact Log Entry - Caller/Party Contacted page as described in one of the above sections, complete the information for each field.  If information has been carried forward because you have linked this contact with a previously entered person or organization, the information in some of the fields will be populated from that linkage.  You may change any information as appropriate.

Case ID

This identification number will be carried forward from a case if a linkage has been made.  Because this is a system-generated field, it cannot be changed.

Organization or Last Name*

Enter the last name of a person or provider or the name of an organization in this field.

First Name

Enter the first name of the person or provider in this field.

Home/Location Address

Enter the first line of the street address in this field.

City

Type the name of the city in this field.

Town

Select the name of the town by using the down arrow () next to the field.

State

The State has been defaulted to Vermont; however if another selection is needed, select the name of the state by using the down arrow () next to the field.

Zip Code

Enter the zip code in this field using the format #####-####.

Contact Number

Enter the telephone number for contact in this field using the format ###-###-####.

Caller/Party Contacted Type*

Select one of the following from the drop-down list using the down arrow next to this field ():

Select one of the following from the drop-down list using the down arrow next to this field ():
