Completing the Enrollment
Once you have selected the Provider and clicked on Continue as described in Continuing with the Enrollment, the Create Certificate page (SB0044) is displayed.
The following information has been forwarded from the Child and Provider records selected.
Child's Name
Age Category
Provider's Name
Provider ID
Eligibility Dates
Complete the information for the following fields to complete the enrollment:
Start Date
Enter the starting date for the enrollment. This date must be within the Eligibility Dates range and this is the pre-filled date in this field.
End Date
Enter the ending date for the enrollment. This date must be within the Eligibility Dates range and this is the pre-filled date in this field.
Variable Schedule
This denotes whether the schedule is fixed or variable for the activity period. To change, select either Yes or No for the selection by using the radio button for this field (

Hours per Week
The hours per week have been calculated by the entries made in Section 5 of 7: Child Care Schedules (SB0030). They may be changed at this time if appropriate.
Relative Care
Select either Yes or No for the selection by using the radio button for this field (). Relative Care indicates if the provider is related to the child in care.
Relative Care Type
Relative Care Type indicates the type of relationship between the child and provider. Select the value for the Family Member by using the down arrow (
) next to the field:
Aunt/Uncle
Grandparent
Great Grandparent
Sibling
FT/PT
Select either Yes or No for the selection by using the radio button for this field (). School Age children who have a subsidy parent with a full time service need may be approved for a FT/PT care level which indicates that the child will be authorized for PT or FT time care and paid for such as submitted on the provider attendance. If this applies, enter a Yes in this field.
