Completing the Transportation Authorization
Once you have selected the Transportation Provider and clicked on Continue as described in Continuing with the Transportation Provider Selection, the New Transportation Authorization page (SB0063) is displayed.
The following information has been forwarded from the Child and Provider records selected.
Child's Name
Age Category
Provider's Name
Provider ID
Eligibility Dates
Complete the information for the following fields to complete the authorization:
Start Date
Enter the starting date for the transportation. This date must be within the Eligibility Dates range.
End Date
Enter the ending date for the transportation. This date must be within the Eligibility Dates range.
Days Authorized
Check each of the days that apply by clicking in the box next to the value:
Sunday
Monday
Tuesday
Wednesday
Thursday
Friday
Saturday
Sunday
To uncheck a selected day, just click in the box again to remove the selection.
Notes
Any appropriate notes for this transportation authorization may be entered in this textbox.
