Conduct Record Check (LP)

A record check is conducted for each individual identified on the submitted Record Check Authorization forms. If a criminal or abuse background is found that would disqualify the applicant from being a provider in accordance with Licensing regulations, the regulatory problem will be used to deny the application.

Provider Portal: When a Record Check is received subsequent to the Internet submission of a Visit Request, the Licensing Technician uses the Case number or other information contained on the form to search for the case and record the record check for the case.
