Creating a New Child Record for Child Care Plan Entry


If you have not been able to locate an existing child's record in the system, you may create a new child record for Child Care Plan entry purposes.  To access this function, go to the Child Care Plans section on the Subsidy main menu.

Click on the Create New Child Record menu option.  As a result, the Create Child Record - Search and Validate Child page (CC0005) is displayed.

Perform a search on this page to ensure that the child has not been created previously in the system.  Enter as much information as you want in the fields presented in order to limit your search results.  The more information entered, the less the results will be returned.

Name (Last*/First)

Enter the name of the child in these fields. 

Search Type

Select one of the following from the drop-down list using the down arrow next to the each of the First and Last name fields () to restrict the search parameters:

Sounds Like

Starts With

Exactly

Contains

Gender

Select Female or Male by clicking in the () to the left of the option.

Date of Birth

Enter the date of birth using the following format MM/DD/YYYY.

Reference ID

Type the identification number to be used with the Reference Type in this field.

Reference Type

If a number has been entered in the previous field, select one of the following for the associated type:

CCIS               CCIS License ID

DOE                 Department of Education Student ID

DMV                Driver's License Number

EIN                  Federal Identification Number

SSN                 Social Security Number

ISN                  PATH ID

SSID                Social Services ID

SEID                State Employee ID

VID                  Vendor ID

 
Once the information has been entered in the above fields, click on the Search button to perform a search in the database for an existing provider.
