Ending the Certificate
There are several reasons for ending a Certificate:
Child Care is no longer needed;
Child is transferring to another provider;
Child or Family is no longer eligible for care;
Client is dissatisfied with provider services; or,
Provider has closed.
To perform this function, select the option named End Certificate from the Additional Options list at the bottom of the Certificate page (SB0056). As a result, the Certificate Detail page (SB0058) is presented.
Complete the following information on this page.
New End Date
Enter the new ending date for the enrollment in this field.
End Reason
Select the reason for ending the enrollment by using the down arrow (
) next to the field to choose the value:
Child Care Services are no Longer Needed
Client Dissatisfaction with Services
Client Ineligible Due to Age
No Longer Eligible for Subsidy
Provider Closed
Transferred to Another Provider
