Enrollment History
The Enrollment History function is provided to view a child's enrollment with child care providers over time. Access the Enrollment History function by clicking on Search Enrollment on the Subsidy Case Management menu options. As a result, the Search Enrollment page (SB0066) is presented for performing a search.
Complete the search criteria in the following fields:
Enrollment ID
If you know the ID Number, enter it here. This entry would give you the most direct connection to the enrollment record.
Child Last Name
Enter the last name of the child in this field.
Child First Name
Enter the first name of the child in this field.
Enrollment Period Between
Enter the date range for the search by using the format MM/DD/YYYY in one or both of the date fields.
Enrollment Status
Select the value for this field by using the down arrow (
) next to the field.
Active
Inactive
Once the information in the above fields has been entered, click on the Search button.
