The Account Summary page (PM0083) contains the following information for each Provider created in the Bright Futures System. The basic information recorded for a Provider appears in the top section of the page.
Provider Name
The complete name of the provider appears in this field.
Provider ID
The system-assigned Provider identification number is displayed.
Provider Type
The Provider type is displayed under the Provider Name. This field contains one of the following values:
Licensed Facilities:
Early Childhood Program
Family Child Care
Non-Recurring Clientele
School Age Care
Registered
Legally Exempt Child Care
Phone
The site telephone number of Provider is displayed in this field.
Facility Address
The site address for the facility is displayed in this field.
Provider Status
The Licensing Status of the provider is displayed in this field.
Days of Operation
The days of operation for the facility is displayed in this field.
Hours of Operation
The hours of operation for the facility is displayed in this field.
Total Capacity
The total capacity of the facility is displayed in this field.
Next Site Visit
The date of the next scheduled site visit for the Provider is displayed in this field.
