Generate Referral Document


The other function located on the Provider Referral Search Results page (RR0013) is to generate the referral document which serves to preserve the search criteria and the results of the search. 

This object generates a document that replicates the search criteria and results for use by the parents in reviewing and selecting a provider. 

      The referral document may be sent as a paper version, as well as, a document that can be used as an e-mail attachment.

      The Referral Document contains a header to indicate that all providers should be researched on the External Portal for information not contained in the referral, i.e., complaints and violations.

      A statement is displayed on the Referral document that directs the parent to the Concern Line for further information if the parent or guardian has no internet access. 

      The following statement should appear at the bottom of the Provider Profile printed document:  "The information in this document has been submitted to the CCSD by the provider.  The provider's profile is shared with you for the purposes of a child care search and most closely matches your stated needs.  In no way does this information represent a recommendation by the CCSD for the provider.  You are encouraged to seek all resources available to you in your search, including:  The Child Care Consumer Concern Line at 1-800-540-7942, and your local Child Care Resource and Referral agency at 1-877-VACCRRA (both are toll-free)."

      No addresses are printed for any providers.  The document and public portal will list the provider's name, town, phone number, and e-mail address if they have agreed to be an e-provider.

 

In order to include providers in the document, click on the box next to the name in the Select column.  To uncheck a selection, just click in the box again to remove it.

Once you have checked the desired providers, click on the Generate Referral Document button in order to complete this function.  As a result, the referral information is captured and you are returned to the Provider Referrals page (RR0028).
