Initial Licensing Visit Request - Cancel Application

If upon review of the list, the provider information is found to be a duplicate of an existing provider, the Cancel Application button can be selected in order to prevent a duplicate entry. This selection accesses a Cancel Application Confirmation page on which the following message is presented:

Warning: If you elect to cancel this application for this Initial Licensing Visit Request, ALL the information you have entered will be deleted.

Do you really wish to delete this information?

Click "Yes" to delete the information. Click "No" to return to the previous page and continue the Initial Licensing Visit Request.

Results of the Cancel Action

If you have selected "Yes", the information that has been entered up to this point will be deleted and no record will remain for selection at a later date.

If you have selected "No", you will be returned to the previous page and have the opportunity to choose either the Change Program Information or Continue option in order to complete the Initial Visit Request.
