Initial Licensing Visit Request - Change Program Information

Upon review of the list, if the provider information is found to have mistakes in the data entry, the Change Program Information button can be selected in order to access the originally entered program information for editing.  Once the information has been edited, the Next button is used to return to the Similar Address Warning page in order to use the Continue option in order to proceed to the next step.
