Intent to Revoke

Record the information on the Change Provider Status page (LI0174) as described in Starting the Provider License Status Change.

Once the information has been entered, click on the Continue button to submit the License Status Change.

For the Intent to Suspend status , the Change Provider License Status - Create Violation page (LI0143) is displayed.

This page presents the opportunity to cite a regulation for the license action.

Note: If any previous violations have been recorded for the provider, the next page displayed is the Change Provider License Status - Regulatory Information page (LI0065).

The Provider Violations table would present the list of violations and a selection may be made from the table by checking the row in the Select column and then by clicking on the Use Selected and Continue button. This violation will become a part of this particular License Action. As a result of this action, the next page displayed is the Change Provider License Status - Create Violations page (LI0143).
