License Action History
This function contains a complete list of all License Actions associated with a provider. The function can be accessed from the Account Summary page under the Account Options. Click on the License Action History link and the License Action History page (LI0252) is displayed.
The information for each action contains the following:
ID Number
Previous License Status
New License Status
License Action Status
Status Change Date
Effective Date
Expiration Date
