Link Site Visit as Investigation

Go to the Actions section at the bottom of the page and click on the Link Site Visit as Investigation option.  As a result, the Link Site Visit page is accessed. 

This page contains a listing of any site visits that have been recorded for a provider.  Within the row for a particular site visit, the Visit Type will be marked Complaint.  Once you have located that row, select that site visit by first clicking on the radio button field () at the beginning of the row in the Select column.  Complete the information described below:

Investigation Activity Status*

Select the correct value for the status from the list presented as you use the down arrow () next to the field.

Completed

In Progress

Investigation Summary

Enter or update any appropriate notes that pertain to this investigation activity.

Then click on the Submit button.  This step returns you to the Linked Site Visit page.  In order to see all associated Site Visits, go the Investigation Log using the sub-nav bar where each linked Site Visit appears as an entry in the table.
