Link/Validate Subject Case

The Process Complaint - Validate Subject Case page allows you to begin the process of linking the subject of the complaint to an existing case in the Bright Futures database.  When this page is accessed, the top of the page contains a box which shows what has been entered as the Subject Information for the complaint.

At the bottom of the page, the Search Results table (page) is presented with either providers for Provider type complaints or People for Subsidy type complaints.  The following information for the people or providers that were found to be a match in the database is displayed so that a match, if any, can be determined for the link.

System ID

Address

Phone Number

E-mail Address

In order to assist in this process, you may access the Provider's or Person's Profile by clicking on the View Profile link at the end of the row in the results table.
