Matching Program Address Warning (LI0008)
If the information entered for the program applicant's address is the same or similar to an address for an existing program in the database, a Warning page is presented which displays the following information.
Program Information Submitted
This text box shows the information that has been entered for which the match was compared.
Provider List
A list is displayed which gives all the similar site addresses as specified in the application. This list presents the following:
Provider Name
Address
If further investigation is required to determine if a duplicate has been found, details for the provider may be accessed for each row in the table by selecting the View Profile option at the end on the row. In addition, the results list may be sorted by using the drop list next to the Sort By field on the top right corner of the table with the following sort options:
Provider Name
Date of Birth
System ID
City, State
Zip Code
Upon review of Program Information Submitted, one of three choices may be made:
Change Program Information
Cancel Application
Continue
