| Term | Definition |
| Fatality | Death of a child including a death that was the result of a serious illness or injury that occurred on the premises of the program, even if the death occurred outside of the program operating hours. |
| Serious Injury | A serious impairment of physical condition, including, but not limited to, the following: loss of consciousness; concussion; bone fracture; protracted loss or impairment of function of any bodily member or organ; a wound requiring extensive suturing; and serious disfigurement. |
| Serious Incident | Any incident or unusual occurrence that is threatening to the health, safety, or welfare of the children, staff or volunteers, including, but not limited to, lost or missing child, illness or injury requiring medical care that was the result of a serious illness or injury that occurred on the premises of the child care, need for emergency responders. |
