New Special Services Agreement

To create a new Special Services entry, select New Special Services Agreement from the Special Services Options at the bottom of the Special Services Agreement page (LI0142). As a result, the New Special Services Agreement page (LI0117) is presented.

On this page, you will associate an agreement with the selected Provider by completing the following fields.

Training Received*

Select one of the following from the list using the radio button next to the field ():

I have already taken 6 hours of Basic Special Services Training this year.

I have taken Basic Special Services Training within the last year and agree to attend 6 hours of Advanced Special Services Training during this year.

I have not yet taken Basic Special Services Training but I agree to attend Basic Special Services Training within the next six months.

I have taken Basic Special Services Training in a prior year and have taken Advanced Special Services Training this year.

Start Date of Training*

Enter the starting date for the training indicated above in this field.

End Date of Training*

Enter the ending date for the training indicated above in this field.

Location(s) of Training*

Enter the training location(s) described above in this field. Separate each location by a comma.

Enter any text needed to describe the incident if appropriate.

This value is system-generated upon submission of this page with a Note included.

Upon completion of the above fields, click on the Submit button. This action results in an entry on the Special Services Agreement page (LI0142).
