New Violation Function

This step proceeds to New Violation page (LI0201) where the Regulation Number and Regulation Text fields are now filled as a result of the selections made from the Regulation Search function.

Complete the following information on this page.

Finding Date

Enter the date when the violation was identified by the Licensor using the MM/DD/YYYY format.

Factual Basis for Violation*

Enter narrative that supports the violation citing, such as 'Offender present in the home at the time of the site visit'.

Status*

The Status for a new entry is system-generated and appears as New.

Notes

You may record any appropriate notes in the Notes field.

Once you have completed the information in the above fields, click on the Update button in order to save this information as part of the License Action. You are then returned to the Change Provider License Status - Create Violation page (LI0143) with the violation entry in the Provider Violations table.
