Adult Abuse Registry Details
On the Adult Abuse Details page, record the following information:
Date of Review*
Enter the date on which the record review is being conducted.
Prohibited Person*
Select Yes or No by using the radio button next to the field (
).
Add Finding
Select one of the following from the drop-down list using the down arrow next to the field (
):
Emotional Abuse
Medical Neglect
Neglect
Physical Abuse
Sexual Abuse
Click on the Add button. This action accesses the Adult Abuse - Finding page. Record the following information for the Finding:
Nature of Offense
This field is populated by the selection made prior to clicking on the Add button.
CDD Worker
This field is populated by the name of the worker who is logged into the system.
Substantiation Date
Enter the date on which the offense was substantiated.
Prohibited Person*
Select Yes or No by using the radio button next to the field ().
Notes
Enter any notes in this field that pertain to the finding.
Click on the Continue button to save the findings and access the page to enter the Date of Review for this finding.
Date of Review*
Enter the date on which the record review is being conducted.
Prohibited Person*
Select Yes or No by using the radio button next to the field ().
