Party Association History
This function allows viewing of any parties that have ever been associated with a particular provider. The list displayed contains both Active and Inactive associations. To access this function, click on the Party Association History link under the Association Options at the bottom of the Associated Party page (PM0004). This link is only available to authorized users of the system. As a result, the Party Association History page (PM0133) is displayed with the information for any person associated with the provider currently or in the past with the following:
Name
System ID Number
Date of Birth
SSN
Dates of Association
Position/Person
