Provider Problem Notice
This system-generated notice allows a Licensor to be notified when a problem exists with a Provider or Provider Organization.
The notice is triggered by the submission of a Site Visit with an outcome of either Violations Noted or Complaint Substantiated.
The Provider Problem Notice is sent to the Licensor responsible for the Provider, if the Licensor did not make the Site Visit, and to the Licensor's Supervisor.
The Provider Problem Notice may also be sent to the Subsidy case workers who have children enrolled with the Provider if the violation(s) found involve parental notification.
In addition a notice is also generated as a result of any corrective actions being entered or updated.
