Provider - Search Criteria (Fees)
When searching the database to determine if a Provider has been previously entered, the object is to use a greater number of search parameters to yield the smallest number of Providers matching the search criteria.
Provider ID
This is a system-assigned Provider Identification Number that may be used as the most direct hit to the provider record.
Program Name or Last Name
Enter the name of the program (Licensed Provider) or the last name of the Provider (Registered or LECC).
Provider First Name
Enter the first name of the Provider (Registered or LECC).
License Number
Enter the license number assigned to the Provider.
Provider EIN/SSN
Type either the EIN or SSN as the identification number to be used in this field.
Town
Select the name of the town by using the down arrow () next to the field.
Provider Type
Select the provider type by using the down arrow () next to the field.
Early Childhood Program
Registered
School Age Care
Non-Recurring Clientele
Family Child Care
LECC
Subsidy
License Status
Select one of the following states from the drop-down list using the down arrow next to the field ():
Applicant
Denied
Expired
Intent to Deny
Intent to Revoke
Intent to Suspend
Licensed
Pending
Revoked
Suspended
Withdrawn
Application Confirmation/ID Number
If you know the Application Confirmation Number, this would also result in a direct hit to the Provider record.
Provider Name Fields - Search Type
Select one of the following from the drop-down list using the down arrow next to the each of the First and Last name fields () to restrict the search parameters:

Contains
Once the information in the above fields has been entered, click on the Search button.
