Alternate Search
If the entries displayed in the Search Results table do not return the expected results, another search should be performed to ensure that the person or provider does not already exist in the database. Use the following search fields available for this purpose:
Last Name or Provider Name
Enter the subject's last name in this field. If the provider is an organization, enter the provider name in the Last Name field.
First Name
Enter the first name of the person that is associated with the Last Name field described above.
