Record New Fair Hearing Request
Once you have clicked on the Record New Fair Hearing Request button on the Fair Hearing Requests page (LI0181), the Record Fair Hearing Request page (LI0084) is displayed.
Enter the following information to process a request for the Provider.
Request Type*
Select the type by using the down arrow () next to the field.
Subsidy
License Action
Enforcement
Date Request Received*
Enter the date on which the request was received.
Reference ID
The Reference ID is given to the Provider in the Record Check Outcome letter and can be entered in this field.
Requestor Type*
Enter the type by whom the request was made by using the radio button next to the field ():
Person
Provider
Person Submitting Request
Enter the name of the person or the provider submitting the request.
Request Narrative
Enter the contents of the request as given by the Provider or Parent.
Notes
Enter any notes in this field that pertain to the request.
