Application Checklist (LECC)


Throughout the LECC Application process, this object provides a tool for tracking the status and acceptability of each section (page LI0249) of the LECC Application and documents the outcome of the application.  Licensing staff track the status and acceptability of each section of the LECC Application using this tool.

Each section of the application is completed separately.  As each section has been completed, it can be assigned a Status.  Upon completion of the entire Application, the Application itself is given a Status as a whole using the Application Submission function.

Note:  If you are creating an Out of State Provider, do not open any sections of the application for which you do not have the required information.  Once a section has been accessed, the required fields will present an edit upon leaving that page.

Section Note
Throughout the completion of each section of the Application, there is a comment area at the bottom of each page for recording any pertinent information about the particular section.
Completion of Each Section

Upon completion of each section of the Application, the navigational buttons at the bottom of the page perform the following functions: 
      Save and Exit - Saves the information that was recorded on the page.
      Application Menu - Saves the information that was recorded on the page and returns to the Application menu page from which another section may be accessed for completion.
      Next - As you complete the information in a section, you may proceed to the next section by clicking the Next button.
      Back - If you want to review a previous section, you may access that section by clicking the Back button.
