Update Provider Schedule and Services

To update schedule and services information:

1. Click the Account Summary tab at the top of the page. Picture

    

2. Click Referral Agreement at the bottom right of the page. Picture

    

3. Click Provider Schedule and Services. Picture

    

4. Click Update Information. Picture

    

5. Enter the required information, and any additional information you want to provide. When complete, click Submit. Picture
