Child A exited the playground gate and made it to the travel portion of the road before staff were able to retrieve the child. It was discovered during the investigation there were identified issues with the gate door properly closing and latching therefore a staff person was assigned to the gate to monitor, however this person was also counted in ratio and by regulation required to provide supervision and attend to children's needs despite the gate assignment. Staff B was attending to a child's needs while standing at the gate when Child A pushed against the gate and it opened allowing them to exit the playground. Staff B requested assistance from Staff C who was also on the playground, and attempted to exit the gate but struggled with getting the gate to open again before being able to retrieve Child A. |
