On 3/31/2020 at approximately 9:20am Infant A- a 14 month old child was lying on his stomach on a carpeted floor in the Infant Room. Infant B- a similarly-aged infant lost her balance and fell unto Infant A's back resulting in Infant A's head hitting the floor. Infant A's top tooth lacerated his bottom gum which caused bleeding. While attending to Infant A's injury, the staff person noticed a top tooth was chipped. The staff person said that Infant A had only been in the program for one day previously and being newly enrolled, the staff person said she did not know if Infant A's tooth had been chipped prior to enrollment or as a result of this injury. The staff person filled out an incident report with oversight provided by the program director. The program director handed the incident report to the parent as she arrived to pick up her son at approximately 2:30pm which was the first time the parent was informed of the injury. The staff person and program director said no attempt was made to contact the parent prior to receiving the incident report which means the program did not notify the parent as quickly as possible which is a violation of this rule.
  Staff shall ensure that if an accident or injury occurs to a child, while in attendance, immediate action is taken to protect the child from further harm and notification of the child's parents as quickly as possible.
