Section 3: Household Members (SB0122)

The Path Household Members table delineates the names, relationships, dates of birth, SSN's and gender of the members of the PATH household. The persons recorded as Application Household Members are those individuals who will be counted in the family size for subsidy eligibility determination. Persons who may also be living in the home but who are not countable for eligibility are not included in this table.

This page is divided into the following sections:

Household Members Over 13

Children up to 13 for non-special needs and up to 19 for Special Needs

Each row in the table contains the following information for each household member:

Name

Relationship

Gender

Date of Birth

SSN

Each of these table entries is added separately and the functionality is the same for all entries.
