Create New Flag


To create a new Red Flag entry, select Create Red Flag from the Red Flag Options at the bottom of the Active Red Flags page.  As a result, the Create Red Flag page (LI0123) is presented. 

 
On this page, you will associate a red flag with the selected Provider by completing the following fields.

Flag Date*

The date field is populated with the current date, but may be changed if necessary.

Type*

Select one of the following from the drop-down list using the down arrow next to the field ():

Failure to Complete Corrective Actions

Number of Parental Notifications

Number of Site Visits with Violations

Number of Violations

Severity of Violations

Other

Notes

Enter any text needed to describe the Red Flag instance if appropriate.

 

Upon completion of the above fields, click on the Submit button.  This action results in a Red Flag being associated with the Provider.  A Red Flag indicator will be displayed on the Account Summary page (PM0083).
