Licensed Program

An application is initiated by the submission of a Initial Licensing Visit Request (Visit Request or VR), accompanied by a Record Check Authorization.  For a new application, the record check authorization must be received in hard copy with a live signature.  The Initial Licensing Visit Request, however, may be submitted from the Internet.  The Internet version will be accompanied by section-by-section instructions for the VR.  In addition, the Internet site will provide a description of the Licensed program process, a copy of the complete application, and instructions for completing the application.  As part of the VR, the applicant will be asked if they would like to become an eProvider, and send licensing forms and communications from the web site.  The system ensures that all required information for the VR is provided before permitting it to be submitted to the CDD Licensing Unit.  Upon completion and submission of the Visit Request, it is Worklisted to a Licensing Technician for review and processing.  In addition, the Case number is assigned by the system and the number is displayed for the provider for use in submission of the hard copy Record Check Authorization.  The Case is open and the License is in an "Applicant" status.

For those who have elected to become an eProvider, following the site visit the applicant completes the Licensed Program Application from their Internet site.  As with the original Site Visit Request, the Internet version will be accompanied by section-by-section instructions for completion of the application.  The system ensures that all required information for the application is provided before permitting it to be submitted to the CDD Licensing Unit.  Upon completion and submission of the application, it is Worklisted to a Licensor for review.
