Record New Variance Request
Once you have clicked on the Record New Variance Request button on the Variance Requests page (LI0118), the Record Variance Request page (LI0137) is displayed.
Enter the following information to process a request for the Provider.
Date Request Received*
Enter the date on which the request was received.
Request Method*
Enter the method by which the request was made by using the radio button next to the field ():
E-mail
Letter
Reference ID
The Reference ID is given to the Provider in the Record Check Outcome letter, and can be entered in this field.
Requestor Type*
Enter the type by whom the request was made by using the radio button next to the field ():
Person
Provider
Person Submitting Request
Enter the name of the person or the provider submitting the request, if the requestor is a person.
Regulation Number*
Type in the Regulation Number associated with the request exactly as it appears in the Regulations.
Regulation Text
This field will be populated with the Regulation Descriptive Text once you click on the Continue button and access the next screen.
Request Narrative
Enter the contents of the request as given by the Provider or Parent.
Notes
Enter any notes in this field that pertain to the request.
Once you have entered the information, the following choices are available:
Validate - Populates the Regulation Number and Text on the Record New Variance Requests page.
Continue- Accesses the Variance Requests Detail page for attachment of a scanned image to the request if available.
