Electronic Contact Information

To ensure you receive correspondence sent by CDD and/or our Community Partners, please keep your e-mail address updated.

To update your electronic contact information:

1. Click the Provider Demographics tab at the top of the page. Picture

   

2. On the far right of your current e-mail or website address, click Update. Picture

   

3. Enter your new electronic contact information. Picture

   

4. Click Save Updates. Picture
